Empowering Low-Income Communities with Health Monitoring in Nairobi
I recently had the opportunity to engage in two days of patient activations nearby two of the “powered by Ilara Health” primary care centers in the peri-urban, low income communities around Nairobi. Ilara Health has partnered with community health volunteers in these areas to offer random blood sugar, blood pressure, weight and height measurements to the community. The visits revealed high numbers of chronic disease patients who are neither on continuous follow-up schedules, nor taking the proper medication. " "" "Our goal during these patient activations is to ensure appropriate patient education, particularly to patients suffering from non-communicable diseases who need continuous monitoring. We refer the patients to our partner clinics for immediate and constant follow-up, including lifestyle modification advice, laboratory investigations, and medication refills." "" "Our hope is to help drive positive healthcare outcomes, even for the most low-income patient groups in Africa.
